Theme: Erotic Massage
Description: A male or female character helping his / her female friend to overcome her constipation using an alternative method by a massage therapy. No enema / laxative to be used by the constipated girl, she has to struggle in passing a huge, hard turd on the toilet after the therapy session
I have two themes to suggest.
Theme: Under Maintenance
Description: Due to routine plumbing maintenance, the water to a school/apartment/office needs to be shut off for several hours leaving the toilets unusable or at least unflushable. This results in one or a number of women having to hold off on their restroom breaks while going about their day. This could lead to accidents, improvised toilets, the embarrassment of using a toilet that cannot flush, trying to find another working toilet or making it just in time when the water returns.
Theme: Unclean
Description: A young woman serves as a nun/priestess in a temple/church that is particularly critical of bodily functions and considers them highly unclean and blasphemous. As such, disciples are forbidden from urinating, defecating or even farting within the temple. Through some set of events the nun/priestess is in dire need of a toilet but is afraid of breaking the rules. This could lead to her either secretly relieving herself or struggling to hold it until she is dismissed for the day. Either way she would have to try to hide how desperate she is from the other disciples and could encounter moments that really test the strength of her bladder/bowels.
Theme: Bound and Desperate
Description: A woman has an accident, or comes very close, because she's tied up. She could be a captive damsel in chains, a hostage tied to a chair, a possessed woman in restraints for her own good, or just an ordinary gal trying out bondage with her lover. As a twist she could deliberately relieve herself because she doesn't want to endure the pain of holding it inside when an accident is inevitable, then she gets untied as soon as her panties are ruined.

Under Maintenance has a lot of potential. Imagine a woman trying to pee in a potted plant or the bushes outside without being seen. Or imagine several women using the same toilet that won't flush. Or suppose one of them has a supply of adult diapers for some reason.
